[Glomerular filtration in low birthweight neonates].

G Boehm, K Beyreiss, W Braun

    Biomedica Biochimica Acta

    Summary

Newborns small for gestational age (SGA) have lower glomerular filtration rates (GFR) initially. GFR increases over three weeks, but remains lower in SGA newborns compared to others.

    Area of Science:

    • Neonatal physiology and nephrology.
    • Pediatric pharmacology and drug dosing.
    • Perinatal medicine and infant development.

    Context:

    • Glomerular filtration rate (GFR) is a key indicator of kidney function in newborns.
    • Gestational age and birth weight significantly influence neonatal physiological parameters.
    • Understanding GFR dynamics is crucial for appropriate medical management of newborns.

    Purpose:

    • To assess and compare the glomerular filtration rate (GFR) in different newborn groups.
    • To track GFR changes during the early postnatal period (2nd day to 3rd week).
    • To evaluate GFR in mature newborns appropriate for gestational age (AGA), mature newborns small for gestational age (SGA), and premature newborns AGA.

    Summary:

    • On the 2nd day of life, GFR was significantly lower in mature SGA newborns (1.21 ml/kg·min) compared to mature AGA (1.84 ml/kg·min) and premature AGA newborns (1.54 ml/kg·min).
    • Over the first three weeks, GFR increased significantly in mature AGA (to 2.20 ml/kg·min) and mature SGA newborns (to 1.65 ml/kg·min).
    • The GFR increase was insignificant in premature AGA newborns (to 1.70 ml/kg·min), resulting in no significant difference between mature SGA and premature AGA newborns by the end of the 3rd week.

    Impact:

    • The diminished GFR in SGA newborns necessitates careful consideration for drug dosages, especially for medications prone to accumulation.
    • Findings inform the composition of parenteral nutrition for SGA newborns to optimize nutrient delivery and excretion.
    • This study highlights the importance of individualized assessment of renal function in neonates based on growth parameters.
