Urinary retention secondary to congenital bladder diverticula in infants
Insights
Giant bladder diverticula are rare causes of infant urinary retention. These congenital anomalies can obstruct the bladder outlet, requiring surgical intervention like bladder diverticulectomy in children.
Area of Science:
- Pediatric Urology
- Congenital Abnormalities
- Urinary Tract Disorders
Background:
- Acute urinary retention is uncommon in infants.
- Congenital bladder diverticula result from localized muscular deficiencies.
- These can occur even with normal voiding pressures.
Observation:
- Two infant cases of enormous bladder diverticula causing posterior compression and bladder outlet obstruction are presented.
- One patient had a solitary, periureteral diverticulum with ipsilateral ureteral reflux.
- The other patient had bilateral paraureteral diverticula without reflux.
Findings:
- Giant bladder diverticula are a rare cause of bladder outlet obstruction in pediatric patients.
- The posterior extension of diverticula can significantly impede urinary flow.
- Congenital diverticula represent a localized mucosal herniation through deficient bladder musculature.
Implications:
- Giant bladder diverticula represent an unusual indication for bladder diverticulectomy in children.
- Early diagnosis and surgical management are crucial for resolving obstruction.
- Understanding the congenital etiology is key for pediatric urologists.
Abstract:
Acute urinary retention in infants is uncommon. We present 2 patients in whom enormous bladder diverticula extended posteriorly, compressing the bladder outlet and causing obstruction. In 1 patient the diverticulum was solitary, periureteral and associated with ipsilateral ureteral reflux, while the other had bilateral paraureteral diverticula without reflux. Primary bladder diverticula in infants and children are the congenital result of localized deficient musculature allowing herniation of mucosa to occur at this site, even with normal voiding pressures. Giant bladder diverticula causing outlet obstruction in small children are rare and constitute one of the unusual indications for bladder diverticulectomy in children.
