    This case study highlights a rare association between primary non-Hodgkin brain lymphoma (PNHBL) and uveitis. Early detection of uveitis preceding neurological signs may offer a better prognosis for PNHBL patients.

    • Primary non-Hodgkin brain lymphoma (PNHBL) is a rare malignancy.
    • Uveitis can be an ocular manifestation of systemic diseases, including lymphomas.
    • The association between PNHBL and uveitis is infrequently reported.

    Observation:

    • A 57-year-old man presented with unexplained posterior uveitis unresponsive to standard treatments.
    • Ocular symptoms preceded the diagnosis of a parieto-occipital PNHBL by nine months.
    • Immunological studies revealed T-lymphocyte deficiency, B-lymphocyte excess, and monoclonal gammopathy (IgM lambda).

    Findings:

    • The patient's monoclonal gammopathy resolved post-neurosurgery.
    • Tumor cells were identified as B-lymphoid type with lambda light chain.
