    • Pediatric Medicine
    • Radiology

    Background:

    • Diffuse interstitial pulmonary disease can present with various complications.
    • Mediastinal emphysema is a rare finding in pediatric patients.

    Observation:

    • A 13-year-old male presented with diffuse interstitial pulmonary disease.
    • The patient subsequently developed mediastinal emphysema.

    Findings:

    • The underlying cause of the pulmonary disease and mediastinal emphysema was diagnosed as sarcoidosis.
    • This represents the first reported case of mediastinal emphysema in a pediatric patient with sarcoidosis.

    Implications:

    • Highlights the potential for rare complications in pediatric sarcoidosis.
    • Suggests the need for vigilance in monitoring for mediastinal emphysema in similar cases.
    • Contributes to the understanding of sarcoidosis manifestations in adolescents.
