    Summary

    Hemorrhage is common in multiple myeloma patients. Plasma exchange effectively treated a patient with myeloma-related bleeding by improving abnormal platelet function.

    Background:

    • Multiple myeloma is a plasma cell malignancy associated with various complications.
    • Hemorrhage complicates the clinical course in approximately 15% of multiple myeloma patients.
    • Bleeding in these patients can occur despite normal coagulation parameters and platelet counts.

    Observation:

    • A patient with multiple myeloma presented with postoperative bleeding and hematuria.
    • Standard coagulation tests and platelet counts were within normal limits.
    • Platelet function studies demonstrated significantly impaired aggregation responses to all tested stimulants.

    Findings:

    • The patient's bleeding abnormalities were successfully treated with plasma exchange.
    • Plasma exchange demonstrated efficacy in reversing both in vitro and in vivo platelet dysfunction.
    • This suggests plasma exchange can be a therapeutic option for managing bleeding complications in multiple myeloma.

    Implications:

    • Plasma exchange may be a valuable therapeutic strategy for managing bleeding in multiple myeloma patients with platelet dysfunction.
    • Understanding and addressing platelet abnormalities are crucial in the management of bleeding complications in multiple myeloma.
