    Summary

Second-look laparoscopy effectively identifies persistent ovarian cancer in patients after chemotherapy. This procedure spares 36.3% of patients from unnecessary surgery by detecting residual disease, including malignant cells in washings.

    Area of Science:

    • Oncology
    • Gynecologic Oncology
    • Surgical Oncology

    Background:

    • Advanced ovarian adenocarcinoma often requires prolonged chemotherapy.
    • Assessing complete remission after treatment can be challenging.
    • Second-look laparotomy has been used to evaluate residual disease.

    Purpose of the Study:

    • To evaluate the utility of second-look laparoscopy.
    • To determine if persistent ovarian cancer can be identified.
    • To assess if patients can be spared second-look laparotomy.

    Main Methods:

    • Prospective study of 22 patients with advanced ovarian adenocarcinoma (FIGO stages IIB-IV).
    • Patients underwent second-look laparoscopy after a median of 23 months of chemotherapy.
    • Evaluation included visual inspection and cytologic washings.

    Related Experiment Videos

    Main Results:

    • Persistent ovarian cancer was found in 8 patients (36.3%) via laparoscopy.
    • Malignant cells in cytologic washings were the sole evidence of disease in 4 patients (18.1%).
    • Absence of visible tumor and malignant cells indicated successful remission.

    Conclusions:

    • Second-look laparoscopy is valuable for detecting residual ovarian cancer.
    • The procedure can spare patients from unnecessary second-look laparotomy.
    • Cytologic analysis is crucial for identifying occult disease.
