    Summary

    Blood gas analyses are not necessary for determining coal miner respiratory disability. Spirometry alone is sufficient for assessing occupational lung impairment, making blood gas tests redundant in disability claims.

    Background:

    • Current guidelines suggest ventilatory capacity assessment alone is insufficient for determining disabling occupational respiratory impairment in coal miners.
    • The Department of Labor routinely requests blood gas analyses for claimants not meeting ventilatory criteria.

    Purpose of the Study:

    • To evaluate the necessity of blood gas analyses in determining pulmonary disability in coal miners.
    • To test the validity of using blood gas studies in conjunction with spirometry for disability assessments.

    Main Methods:

    • Two groups of coal miners claiming total disability were studied.
    • Group 1 (150 claimants) underwent spirometry.
    • Group 2 (50 claimants) underwent blood gas studies.

    Main Results:

    • Only eight claimants in Group 1 met disability criteria.
    • Only two claimants in Group 2 met disability criteria.
    • In both groups, identified disability was attributed to cardiac disease, not solely respiratory impairment.

    Conclusions:

    • Blood gas analyses are unnecessary for determining pulmonary disability in coal miners.
    • Spirometry appears adequate for assessing occupational respiratory impairment in this population.
    • The routine use of blood gas studies in disability claims may not be justified.
