Gonococcal endocarditis associated with immune complex glomerulonephritis.

J R Ebright, R Komorowski

    The American Journal of Medicine
    |May 1, 1980
    PubMed
    Summary

Chronic gonococcemia can lead to severe kidney damage, causing nephrotic syndrome and immune-complex glomerulonephritis. This case highlights the renal complications of this persistent gonococcal infection.

    Area of Science:

    • Nephrology
    • Infectious Diseases
    • Immunology

    Background:

    • Chronic gonococcemia is a rare but serious manifestation of Neisseria gonorrhoeae infection.
    • Endocarditis is a known complication, but renal involvement is less frequently reported.
    • Immune-complex glomerulonephritis can arise from various systemic infections.

    Observation:

    • A patient presented with symptoms suggestive of chronic gonococcemia.
    • The patient developed endocarditis and nephrotic syndrome.
    • Kidney biopsy revealed diffuse immune-complex glomerulonephritis.

    Findings:

    • This is the 11th reported case of gonococcal endocarditis since 1948.
    • The study provides the first detailed immunofluorescence and electron microscopy of kidney tissue in gonococcal endocarditis.
    • Findings indicate immune-complex nephritis as the cause of the patient's declining renal function.

    Implications:

    • This case underscores the potential for chronic gonococcemia to cause significant renal pathology.
    • Early diagnosis and treatment of gonococcal infections are crucial to prevent severe systemic complications.
    • Further research is needed to understand the pathogenesis of gonococcal-induced glomerulonephritis.
