    Background:

    • Chondroplasia punctata (CP) is a rare skeletal dysplasia.
    • Previous research suggests CP is a heterogeneous condition.
    • Understanding its varied manifestations is key for diagnosis.

    Observation:

    • Two patients with chondroplasia punctata exhibited significantly different disease courses and symptoms.
    • Patient 1 displayed severe rhizomelic shortening, calcifications, cataracts, and a fatal outcome.
    • Patient 2 had milder Conradi-Hünermann-type CP, surviving with less severe skeletal and ocular changes.

    Findings:

    • The observed cases support the classification of CP into two distinct types: rhizomelic and Conradi-Hünermann.
    • Rhizomelic CP is characterized by severe limb shortening, calcifications, cataracts, and often a fatal prognosis.
    • Conradi-Hünermann CP presents with less severe skeletal abnormalities and a better survival rate.

    Implications:

    • Accurate classification of chondroplasia punctata subtypes is essential for predicting prognosis.
    • Early and thorough radiological assessment in infancy is critical for distinguishing between CP types.
    • Further research into the genetic underpinnings of CP heterogeneity may reveal targeted therapeutic strategies.
