    Summary

    Long-term heart problems can occur after mitral valve replacement due to valve prosthesis-patient mismatch. This mismatch can lead to progressive cardiac enlargement and pulmonary hypertension, even with normal prosthetic valve function.

    Background:

    • Mitral valve replacement (MVR) is a common procedure for severe mitral valve disease.
    • Prosthetic valve function is typically assessed by hemodynamic parameters.

    Observation:

    • A patient with an MVR from 1966 developed symptoms and progressive cardiac enlargement nine years later.
    • Despite excellent initial results, the patient presented with functional class II and massive cardiomegaly, including left atrial, right ventricular, and right atrial enlargement.

    Findings:

    • Cardiac catheterization revealed normal prosthetic valve function.
    • Moderate exercise induced severe left atrial and pulmonary arterial hypertension.

    Implications:

    • This case highlights a potential long-term complication of prosthetic heart valves: valve prosthesis-patient mismatch.
    • Mismatch can lead to significant cardiovascular sequelae, including pulmonary hypertension, despite seemingly normal valve function.
