    Summary

    New urinary metabolites, 4-hydroxyisovaleric acid and mesaconic acid, were identified in isovaleric acidemia. This finding supports the metabolic pathway of isovaleric acid and suggests further breakdown of methylsuccinic acid.

    Observation:

    • Two novel metabolites, 4-hydroxyisovaleric acid and mesaconic acid, were detected in patient urine.
    • 3-methylbutyrolactone, a derivative of 4-hydroxyisovaleric acid, was observed in volatile profiles.

    Findings:

    • The presence of 4-hydroxyisovaleric acid supports the omega-oxidation of isovaleric acid to methylsuccinic acid.
    • Mesaconic acid identification suggests further metabolism of methylsuccinic acid in isovaleric acidemia.

    Implications:

    • These findings expand the known metabolic profile of isovaleric acidemia.
