[Current concepts of the drug therapy of glomerulonephritis]
Insights
Chronic glomerulonephritis treatment has yielded poor results, necessitating new approaches. Advances in understanding immune responses offer hope for more effective, selective immunotherapies.
Area of Science:
- Nephrology
- Immunology
- Pharmacology
Context:
- Chronic glomerulonephritis (CGN) treatment over two decades has shown unsatisfactory outcomes.
- CGN is a leading cause of end-stage renal disease, requiring dialysis and transplantation.
- Therapeutic success assessment is hindered by inconsistent methodologies and criteria.
Purpose:
- To review existing treatment strategies for chronic glomerulonephritis.
- To highlight the challenges in evaluating therapeutic efficacy.
- To explore the potential of novel immunotherapies based on recent immunological insights.
Summary:
- Analysis of 32 studies reveals significant variability in treatment protocols and outcome assessments for CGN.
- Current therapeutic approaches have not adequately addressed the economic burden and clinical progression of CGN.
- Emerging knowledge of immune mechanisms and inflammatory mediators provides a foundation for improved treatment.
Impact:
- Future therapies may achieve more consistent and selective immunotherapy for chronic glomerulonephritis.
- Standardized treatment and evaluation criteria are needed for better therapeutic comparisons.
- Advancements could reduce the incidence of dialysis and transplantation for CGN patients.
Abstract:
The concept of treatment of chronic glomerulonephritis, practiced more than 2 decenniums led to unsatisfying results. All over the world the chronic glomerulonephritis is the most expensive in economic respect renal disease. It is still the most frequent indication to dialysis and transplantation. The judgment of the success of therapy was rendered difficult by several factors. Different nomenclatures, inhomogeneous dosages and durations of therapy, inhomogeneous judgments of the beginning of therapy, of the degree of activity and the judgment of effectivity render the comparison difficult. The results from 32 studies are given in form of tables. Newer knowledge of the immune state and of the mediators of inflammation will broaden the therapeutic basis in a generally accepted immunogenesis of the chronic glomerulonephritis and will lead to an equalized selective immunotherapy.
