    A new, smaller laparoscopic accessory set and photographic technique improve safety and ease of use. This system enabled tissue sampling in over 400 high-risk patients under local anesthesia without serious complications.

    • To develop and evaluate a novel, smaller second puncture accessory set for laparoscopy.
    • To introduce an improved photographic technique for enhanced laparoscopic imaging.

    • A 4mm trocar with biopsy accessories, suction-coagulation cannula, and insulated grasper was utilized.
    • A presterilized quartz rod with a sealed flash tube, synchronized with a camera, was employed for photography.
    • The system was tested in over 400 high-risk patients under local anesthesia.

    • The smaller trocar facilitated safe and easy introduction and manipulation.
    • Tissue samples were successfully obtained without serious complications or fatalities.
