    Background:

    • Asbestosis is classically linked to restrictive lung function patterns.
    • Peripheral airway obstruction, often missed by spirometry, is also a common finding in asbestosis.
    • Peribronchiolar fibrosis may underlie peripheral airway obstruction in asbestos-exposed individuals.

    Purpose of the Study:

    • To investigate the impact of asbestos exposure on lung function in a cohort of workers.
    • To compare lung function changes between smokers and nonsmokers exposed to asbestos.
    • To identify sensitive measures for detecting asbestos-related lung disease.

    Main Methods:

    • Maximal expiratory flow volume (MEFV) curves, closing volume, and alveolar plateau slope were analyzed.
    • Data from 610 asbestos-exposed workers (575 men, 35 women) were stratified by smoking status and years worked.
    • Lung function parameters were compared to predicted values and analyzed for rates of change.

    Main Results:

    • Both smokers and nonsmokers showed significantly increased closing capacities and delta N2% (alveolar plateau slope), and decreased FVC, FEV1.0, MEFR, and PEFR compared to predicted values.
    • FEF75% was significantly reduced only in workers with over 30 years of exposure.
    • Smokers exhibited more rapid declines in FVC, FEV1.0, PEFR, MEFR, and FEF75% compared to nonsmokers.

    Conclusions:

    • Closing capacity and alveolar plateau slope are sensitive indicators of asbestos-induced lung function impairment.
    • Asbestos exposure affects lung function in both smokers and nonsmokers, with smoking exacerbating the decline.
    • Peripheral airway changes, detectable by closing volume and alveolar plateau slope, are significant in asbestos-exposed individuals.
