Electron microscopic changes in tropical endomyocardial fibrosis.

H Krishnaswami, A Date, A Bhaktaviziam

Transactions of the Royal Society of Tropical Medicine and Hygiene
    |January 1, 1984

    Summary

Electron microscopic analysis of cardiac tissue reveals that tropical endomyocardial fibrosis involves granulation tissue formation and myofiber damage. These pathological changes in the endocardium and cardiac muscle resemble those seen in eosinophilic heart disease.

    Area of Science:

    • Cardiovascular Pathology
    • Electron Microscopy
    • Tropical Medicine

    Background:

    • Tropical endomyocardial fibrosis is a cardiac condition affecting tropical regions.
    • Previous studies have described characteristic pathological findings in affected cardiac tissue.

    Purpose of the Study:

    • To investigate the ultrastructural changes in cardiac tissue from patients with tropical endomyocardial fibrosis using electron microscopy.
    • To compare the observed pathological features with those of other eosinophilic heart diseases.

    Main Methods:

    • Cardiac tissue samples from seven patients with tropical endomyocardial fibrosis were examined using electron microscopy.
    • Detailed analysis of endocardial, myofiber, and small blood vessel morphology was performed.

    Related Experiment Videos

    Main Results:

    • The endocardium was replaced by granulation tissue with collagen, smooth muscle cells, blood vessels, and mast cells.
    • Myofibers exhibited mitochondrial swelling, lamellar bodies, and myocytoplasmic clearing.
    • Small blood vessels showed endothelial swelling, basement membrane duplication, and leukocyte infiltration, including eosinophils with altered granule morphology.

    Conclusions:

    • The electron microscopic findings in tropical endomyocardial fibrosis demonstrate significant endocardial and myocardial injury.
    • The observed pathological changes share similarities with lesions reported in endomyocardial disease associated with eosinophilia, suggesting a potential link or common pathway.
