    Summary

    Surgical removal of parathyroid glands resolved hypercalcemia and prolonged fever in a patient with hyperparathyroidism. This intervention normalized both biochemical markers and body temperature.

    Background:

    Observation:

    • A 36-year-old woman presented with symptomatic hypercalcemia, hypercalciuria, elevated parathyroid hormone, and persistent fever.
    • These clinical and biochemical findings suggested a diagnosis of primary hyperparathyroidism.

    Findings:

    • Surgical excision of parathyroid adenomatous and hyperplastic glands was performed.
    • Post-surgery, the patient experienced a complete resolution of hypercalcemia and hypercalciuria.
    • Normalization of serum parathyroid hormone levels and body temperature was observed.

    Implications:

    • Surgical management is effective for symptomatic primary hyperparathyroidism.
    • Addressing parathyroid gland abnormalities can resolve associated systemic symptoms like fever.
    • This case highlights the importance of considering endocrine disorders in the differential diagnosis of unexplained fever and hypercalcemia.
