    Summary

Exercise-induced wheezing in a patient was caused by vocal cord collapse during inspiration. Symptoms improved with physical conditioning and nasal steroid treatment for rhinitis.

    Area of Science:

    • Pulmonology
    • Exercise Physiology
    • Laryngology

    Background:

    • A patient experienced a decade of exercise-induced wheezing.
    • Initial treatments with metaproterenol and cromolyn were ineffective.

    Observation:

    • During exercise observation, inspiratory stridor was noted.
    • This stridor resulted from posterior aryepiglottic fold collapse over the vocal cords during inspiration post-exercise.

    Findings:

    • Physical conditioning was the primary treatment for symptom improvement.
    • Steroid nasal spray for chronic rhinitis and post-nasal drip provided additional benefit.

    Implications:

    • Suggests a mechanical cause for exercise-induced wheezing in some cases.
    • Highlights the importance of addressing upper airway conditions like rhinitis.
    • Physical conditioning may be a key therapeutic strategy for this specific condition.
