[Problems for discussion in clinical diabetology]

According to the literature data and his own experience the author considers some problems concerning the epidemiology, pathogenesis, clinical criteria, diagnosis and treatment of diabetes mellitus. First of all, it is appropriate to examine persons with factors of diabetic risk to perform active dispensarization and to manage early diabetes mellitus forms. The role of the peripheral nervous system in the disease pathogenesis is also discussed. Additional criteria for determining the severity and the degree of diabetes mellitus compensation are proposed for the development of a relative classification of the disease. The given methods for prediabetes diagnosis necessitate a wide clinical study. The data are provided in favor of a different approach to the treatment of diabetic ketoacidosis and ketoacidotic coma. An original conception of the disease development is presented.
