    Summary

    Thyroid function tests are unreliable in patients taking amiodarone. Elevated thyroxine levels and suppressed TSH secretion are common, but amiodarone-induced hyperthyroidism is rare.

    Purpose:

    • To investigate thyroid function in patients on chronic amiodarone treatment.
    • To assess the reliability of standard thyroid function screening parameters.

    Summary:

    • Elevated total and free thyroxine levels were observed in 27% of patients, with suppressed TSH secretion in 15%.
    • Triiodothyronine levels were mostly low or below normal.
    • Only one patient exhibited symptoms of amiodarone-induced hyperthyroidism, with thyroid function normalizing post-discontinuation.

    Impact:

    • Standard thyroid function tests (total and free thyroxine) are not reliable for monitoring patients on chronic amiodarone.
