Background:

Carotid artery-cavernous sinus fistula (CCSF) is a rare but serious complication typically resulting from severe head trauma.
Early diagnosis and intervention are crucial for managing CCSF and preventing severe sequelae.

Observation:

CCSF presents with characteristic symptoms including pulsatile headache, bruit, and visual disturbances.
Physical examination may reveal bruit, conjunctival engorgement, chemosis, proptosis, and ophthalmoplegia.

Findings:

A variety of surgical techniques exist for CCSF repair, including cervical ligation and endovascular balloon occlusion.
Two detailed case reports illustrate clinical findings, treatment strategies, and long-term outcomes for CCSF.

Implications:

Maxillofacial surgeons must be adept at diagnosing and initiating treatment for CCSF.
Otolaryngologists should possess knowledge of the vascular dynamics and surgical interventions for traumatic CCSF, even if surgical treatment is outside their direct scope.
