Endobronchial tuberculosis is rarely diagnosed today, often mimicking lung cancer. Fiberoptic bronchoscopy is crucial for diagnosing this uncommon presentation of tuberculosis.

    Area of Science:

    • Pulmonology
    • Infectious Diseases
    • Oncology

    Background:

    • Endobronchial tuberculosis was common before chemotherapy but is now seldom recognized.
    • Declining tuberculosis incidence alters its clinical and radiographic presentation in the US.

    Observation:

    • Four patients presented with symptoms suggestive of primary bronchogenic carcinoma.
    • Three patients had upper lobe disease with segmental collapse.

    Findings:

    • Fiberoptic bronchoscopy confirmed endobronchial tuberculosis in all four patients.
    • Atypical clinical and radiographic features can mask tuberculosis.

    Implications:

    • Tuberculosis diagnosis should be considered even with atypical presentations.
