    Summary

    Juvenile rheumatoid arthritis can present with leukopenia and thrombocytopenia, which are previously unreported findings. These blood count changes should not rule out a diagnosis of Still's disease in affected patients.

    Background:

    • Juvenile rheumatoid arthritis (JRA), now known as juvenile idiopathic arthritis (JIA), is a chronic autoimmune disease affecting children.
    • Typical JRA manifestations include joint inflammation, fever, and rash.
    • Hematologic abnormalities are not commonly reported as primary findings in JRA.

    Observation:

    • Two patients, a 14-year-old female and a 20-year-old male, presented with classic symptoms of JRA.
    • Both patients exhibited concurrent leukopenia (low white blood cell count) and thrombocytopenia (low platelet count).

    Findings:

    • Leukopenia and thrombocytopenia are identified as previously unreported co-occurring findings in patients with JRA.
    • The study highlights these hematologic abnormalities in the context of typical JRA clinical presentation.

    Implications:

    • The presence of leukopenia should not be a reason to exclude a diagnosis of Still's disease (a subset of JRA).
    • Clinicians should consider a broader range of hematologic presentations in JRA.
    • Further research is warranted to understand the mechanism and significance of these hematologic findings in JRA.
