    Parathyroid carcinoma is a rare cause of primary hyperparathyroidism, found in 4.4% of patients. Most cases lacked preoperative signs of malignancy, highlighting diagnostic challenges.

    • Primary hyperparathyroidism (pHPT) is a condition characterized by excessive parathyroid hormone secretion.
    • Parathyroid carcinoma is an exceptionally rare malignancy, often presenting diagnostic difficulties.

    Observation:

    • A 10-year study at University Hospitals of Ulm and Heidelberg included 117 patients with pHPT.
    • Five cases of parathyroid carcinoma were identified within this cohort.

    Findings:

    • The incidence of parathyroid carcinoma was 4.4% among the studied pHPT patients.
    • Crucially, four out of five patients with parathyroid carcinoma showed no preoperative clinical indicators of malignancy.
