    Summary

Hidden-loop colostomies offer a valuable option for patients with metastatic colon cancer and impending obstruction. This technique allows for simpler, emergency room-based opening, avoiding major surgery and extending stoma-free time.

    Area of Science:

    • Surgical Oncology
    • Gastroenterology

    Background:

    • Metastatic colon cancer frequently presents with impending bowel obstruction.
    • Management of obstruction in advanced cancer often requires surgical intervention.

    Purpose of the Study:

    • To evaluate the efficacy and safety of hidden-loop colostomy in patients with metastatic colon cancer and impending obstruction.
    • To assess the feasibility of opening these colostomies in an emergency room setting.

    Main Methods:

    • Retrospective review of 15 patients who underwent a hidden-loop colostomy for metastatic colon cancer.
    • Analysis of colostomy opening procedures, anesthesia used, and patient outcomes.

    Main Results:

    • Six patients required subsequent colostomy opening due to cancer-related obstruction.
    • All openings were successfully performed under local anesthesia in the emergency room.
    • This approach averted six major celiotomies and provided additional stoma-free survival.
    • Two cases of postoperative stomal prolapse occurred, with one requiring reoperation.

    Conclusions:

    • Hidden-loop colostomy is a straightforward and easily opened procedure.
    • It is a viable option for selected patients with metastatic colon cancer and impending obstruction.
    • Consideration at the time of celiotomy can improve patient management and quality of life.
