    Summary

    Background:

    • Unilateral ventrolateral thalamotomy (VLT) is a surgical procedure used to treat tremors.
    • The effects of VLT on visual searching and spatial attention are not fully understood.
    • Previous research suggests potential links between thalamic lesions and attentional deficits.

    Purpose of the Study:

    • To investigate the impact of unilateral left and right ventrolateral thalamotomy (LVLT and RVLT) on visual searching abilities.
    • To determine if VLT induces hemi-inattention or other visual search deficits.
    • To explore the relationship between VLT-induced hemi-inattention and motor neglect.

    Main Methods:

    • Twenty-one patients with essential or Parkinsonian tremor underwent unilateral VLT (10 LVLT, 11 RVLT).
    • Visual searching for parallel lines was assessed before and after surgery.
    • Performance was analyzed for directional biases and search impairments.

    Main Results:

    • RVLT resulted in impaired visual searching predominantly on the left side.
    • LVLT led to bilateral visual searching impairment, more pronounced on the right.
    • A tendency to initiate visual search from the ipsilateral side of the lesion increased post-VLT.

    Conclusions:

    • Unilateral VLT can induce hemi-inattention in visual searching.
    • The observed visual search deficits may be linked to transient motor neglect contralateral to the VLT.
    • These findings highlight the role of the thalamus in spatial attention and visual processing.
