Bone lesions in systemic lupus erythematosus.

    Summary

    Systemic lupus erythematosus (SLE) can cause cystic bone lesions in joints, particularly in the hands and feet. Vasculitis-induced nutritional bone disturbance is a potential cause for these common radiographic findings.

    Background:

    • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease.
    • Articular symptoms are common in SLE patients.
    • Radiographic changes in SLE joints require further characterization.

    Purpose of the Study:

    • To investigate the prevalence and characteristics of cystic bone lesions in patients with SLE.
    • To explore potential pathogenetic mechanisms for these lesions.

    Main Methods:

    • Retrospective review of routine joint radiographs from 125 patients diagnosed with SLE.
    • Correlation of radiographic findings with clinical history of articular symptoms.

    Main Results:

    • 121 out of 125 patients (97%) had a history of joint symptoms.
    • Cystic bone lesions were identified in 51 patients (41%).
    • Lesions were predominantly subchondral in the small joints of the hands and feet, appearing as well-defined radiolucent areas.

    • The characteristic location and appearance suggest a specific pathogenesis.
    • Vasculitis-related nutritional disturbance of bone is proposed as a likely mechanism for lesion development.
