    Background:

    • Thrombocytopenia is a known complication of heparin therapy.
    • Limited research exists on thrombocytopenia incidence with low-dose heparin for deep venous thrombosis (DVT) prophylaxis.

    Purpose of the Study:

    • To determine the incidence of thrombocytopenia in patients receiving low-dose heparin for DVT prophylaxis.
    • To assess the clinical significance of thrombocytopenia in this patient population.

    Main Methods:

    • Prospective study of 66 courses of low-dose heparin (10,000-15,000 units/day) for DVT prophylaxis in coronary care unit patients.
    • Monitoring of platelet counts and clinical outcomes, including thrombosis, bleeding, and fibrin split products.

    Main Results:

    • Mild thrombocytopenia (platelet count < 150 X 10(3)/mm3) occurred in 15% of heparin prophylaxis courses.
    • Thrombocytopenic patients had significantly lower initial platelet counts.
    • Platelet counts normalized despite continued heparin therapy in all cases; no severe thrombocytopenia or clinical sequelae were observed.

    Conclusions:

    • Mild thrombocytopenia is a common, yet clinically insignificant, finding in patients receiving low-dose heparin for DVT prophylaxis.
    • The condition typically occurs 2-5 days after initiation of therapy and resolves without intervention.
