    Background:

    • Mental symptoms are common in myotonic dystrophy, but brain pathology is not well-documented.
    • Central nervous system involvement requires further investigation using advanced imaging techniques.

    Purpose of the Study:

    • To assess central nervous system changes in myotonic dystrophy patients.
    • To investigate brain alterations using CT scans and electroencephalography (EEG).

    Main Methods:

    • CT scans and EEGs were performed on 14 myotonic dystrophy patients (10 males, 4 females; ages 15-50).
    • Ventricular size was compared to 49 neurologically normal individuals.
    • EEG findings were analyzed for background activity and wave patterns.

    Main Results:

    • CT scans revealed diffuse enlargement of lateral and third ventricles without significant cerebral cortex or brainstem atrophy.
    • Ventricular enlargement showed a significant positive correlation with age (P < 0.01).
    • EEG showed slowing and low-voltage background activity; 4 patients had monotonous alpha waves. No asymmetry or paroxysmal waves were detected.

    Conclusions:

    • Ventricular enlargement in myotonic dystrophy is primarily associated with aging, not disease severity or mental decline.
    • This suggests a progressive neuropathology, potentially affecting white matter.
