    Background:

    • Multicentric reticulohistiocytosis (MRH) is a rare multisystemic non-Langerhans cell histiocytosis.
    • Association with other autoimmune or inflammatory conditions is occasionally reported.

    Observation:

    • A 49-year-old male presented with clinical features of multicentric reticulohistiocytosis.
    • The patient also had concurrent polymyositis, a rare inflammatory myopathy.
    • Clinical presentation, light microscopy, and electron microscopy findings were analyzed.

    Findings:

    • Azathioprine treatment led to partial improvement in the skin manifestations of MRH.
    • The patient's general systemic condition did not show significant change despite treatment.

    Implications:

    • This case highlights the complex presentation of MRH, particularly when associated with polymyositis.
    • Further research into optimal therapeutic strategies for MRH, especially in the context of co-existing inflammatory diseases, is warranted.
