    Anterior mandibulectomy for head and neck tumors may cause sleep apnea. Reconstruction of the jaw after surgery appears to prevent this complication, unlike lower lip suspension.

    Background:

    • Extensive anterior oral cavity resections for head and neck tumors often involve anterior mandibulectomy.
    • These resections can significantly alter anatomy, potentially impacting airway function.

    Observation:

    • Patients undergoing anterior mandibulectomy for head and neck tumors are at risk for developing sleep apnea.
    • Tracheostomy is common in these patients, requiring evaluation before decannulation.

    Findings:

    • Subsequent flap or rigid reconstruction of the lower jaw following anterior mandibulectomy appears to prevent the development of sleep apnea.
    • Fascial sling suspension of the lower lip does not seem to prevent post-surgical sleep apnea.

    Implications:

    • Early sleep apnea evaluation is crucial for head and neck cancer patients post-resection.
    • Mandibular reconstruction strategies should consider airway protection to mitigate sleep apnea risk.
    • Further research into optimal reconstructive techniques for preventing sleep apnea is warranted.
