    Isolated iliac aneurysms are rare but have a high rupture rate (37.5%) compared to abdominal aortic aneurysms. Males are more commonly affected, and rupture significantly increases mortality risk.

    Background:

    • Isolated iliac aneurysms (IIAs) are uncommon, with a relative incidence to abdominal aortic aneurysms (AAAs) of 0.9-1.9%.
    • This study analyzes 20 IIAs in 16 patients treated at a vascular service.

    Observation:

    • The relative incidence of IIAs to AAAs was found to be 4.7% in this cohort.
    • Seventeen aneurysms involved the common iliac artery, two the internal, and one the external iliac artery.
    • The majority of patients (15/16) were male, indicating a male preponderance.

    Findings:

    • A significant rupture rate of 37.5% was observed for IIAs, considerably higher than the 11.1% for AAAs.
    • Ruptured IIAs had a high postoperative mortality rate of 60%.
    • Common iliac artery aneurysms were associated with iliac arteriovenous fistulas in two cases.
