    Summary

    Routine screening for hypergastrinemia in primary hyperparathyroidism patients offers minimal clinical benefit. Elevated gastrin levels in these patients often resolve post-parathyroidectomy or are unrelated to the condition.

    Background:

    • Primary hyperparathyroidism is a common endocrine disorder.
    • Hypergastrinemia, elevated gastrin levels, can be associated with various gastrointestinal and endocrine conditions.
    • The relationship between primary hyperparathyroidism and hypergastrinemia requires further investigation.

    Purpose of the Study:

    • To determine the prevalence of hypergastrinemia in patients with primary hyperparathyroidism.
    • To investigate the impact of parathyroidectomy on gastrin levels.
    • To assess the clinical significance of hypergastrinemia in this patient population.

    Main Methods:

    • Serum calcium, parathyroid hormone, and gastrin levels were measured in 38 patients with primary hyperparathyroidism.
    • Gastrin levels were assessed pre- and post-parathyroidectomy.
    • Gastrin levels were evaluated in response to secretin and meal challenges in select cases.

    Main Results:

    • Three patients (7.9%) presented with grossly elevated gastrin levels; two were achlorhydric.
    • Following parathyroidectomy, one patient's gastrin levels decreased but remained elevated.
    • Eight patients (21%) had modest hypergastrinemia, with levels normalizing in three after parathyroidectomy; one patient had rheumatoid arthritis.
    • No correlation was found between parathyroid hormone or calcium levels and gastrin levels.
    • Patients with duodenal ulcers did not exhibit elevated gastrin levels.

    Conclusions:

    • Routine screening for hypergastrinemia in primary hyperparathyroidism patients provides limited clinical utility.
    • Hypergastrinemia in primary hyperparathyroidism may be transient or unrelated to the endocrine disorder.
    • Further investigation is warranted for complex cases involving adrenal or islet cell abnormalities.
