    Background:

    • Eccrine epithelioma is a rare malignant adnexal tumor originating from the sweat glands.
    • Understanding its histopathological features and clinical behavior is crucial for diagnosis and management.

    Observation:

    • A 62-year-old female presented with a 36-year history of a left forearm lesion.
    • The lesion exhibited multiple local recurrences and demonstrated aggressive local invasion into surrounding fat and muscle tissues.

    Findings:

    • Histopathology revealed nests of basaloid cells with cystic and syringoid epithelial structures.
    • Tumor components contained periodic acid-Schiff (PAS)-positive, diastate-resistant, and alcian blue-positive material, indicative of glandular differentiation.
    • Despite aggressive local behavior and recurrence, no distant metastasis was observed.
