    Summary

The Hoffmann-Vidal external fixation frame can be reused 4-5 times if components are inspected and screws replaced after each use. Proper T-wrench tightening prevents overloading and maintains screw/rod integrity.

    Area of Science:

    • Orthopedic biomechanics
    • Biomaterials science
    • Medical device engineering

    Context:

    • The reusability of the Hoffmann-Vidal external fixation frame is debated without sufficient scientific evidence.
    • Orthopedic surgeons require data on the long-term performance and safety of reusable fixation devices.

    Purpose:

    • To investigate the impact of repeated use on the stiffness and fatigue life of Hoffmann-Vidal frame components.
    • To identify failure modes and assess the mechanical integrity of the external fixator after simulated clinical use.

    Summary:

    • Failures occurred at the pin fixation bracket rod, including complete/incomplete fractures and loosening. Complete fractures significantly reduced frame stiffness.
    • Screw fixation strength decreased by 16% after 2.5 million cycles, with deformation and fractures observed in retaining and fixation screws after repeated tightening.
    • Wear debris in articulation couplings and flattened screw threads indicate material degradation. No gross defects were found on fracture surfaces.

    Impact:

    • The Hoffmann-Vidal frame is suitable for 4-5 sequential applications with thorough inspection and screw replacement after each use.
    • Appropriate T-wrench tightening and periodic retightening are crucial to prevent component failure and wear.
    • This study provides evidence-based guidelines for the safe and effective reuse of external fixation devices, optimizing patient outcomes and resource management.
