    Aneurysmal bone cysts in the temporal bone (os temporale) can be treated with subtotal removal, preventing recurrence. Biopsy is crucial for diagnosis, as imaging can be misleading for this benign bone lesion.

    Background:

    • Aneurysmal bone cysts (ABCs) are rare, benign, osteolytic bone lesions.
    • The os temporale (temporal bone) is an uncommon site for ABCs.
    • Accurate diagnosis of ABCs is challenging due to overlapping clinical and radiological features with neoplasms.

    Observation:

    • Two cases of aneurysmal bone cysts in the os temporale are presented.
    • Patients underwent subtotal removal of the cystic lesions.
    • Post-operative follow-up showed no disease recurrence at 20 months and 5 years.

    Findings:

    • Subtotal resection of temporal bone ABCs can be an effective treatment modality.
    • Histopathological examination of the cyst wall is essential for definitive diagnosis.
    • Differential diagnosis must include various bone neoplasms.

    Implications:

    • Early and accurate diagnosis through biopsy is vital for appropriate management of os temporale ABCs.
    • Surgical intervention, such as subtotal removal, offers a favorable prognosis for these benign but potentially recurrent lesions.
    • This study contributes to understanding the management of rare aneurysmal bone cysts in the temporal bone.
