    Summary

    An adrenal tumor secreting gastrin caused intractable diarrhea and high stomach acid. Surgical removal of the tumor resolved the patient's symptoms, normalizing gastrin and acid levels.

    Background:

    • Intractable diarrhea can be a symptom of rare endocrine disorders.
    • Hypergastrinemia and hyperchlorhydria indicate excessive gastrin and stomach acid production.

    Observation:

    • A patient presented with severe diarrhea, elevated serum gastrin, and increased gastric acid.
    • Secretin stimulation test showed an abnormal increase in serum gastrin.
    • Abdominal surgery revealed a large adrenal adenoma, with no pancreatic abnormalities.

    Findings:

    • The adrenal adenoma contained gastrin levels 1,000 times higher than normal.
    • Post-adrenalectomy, the patient's diarrhea resolved, and gastrin and gastric acid levels normalized.

    Implications:

    • This case suggests adrenal tumors can secrete gastrin, leading to gastrointestinal symptoms.
    • It highlights the importance of considering ectopic hormone production in unexplained clinical presentations.
    • Further research may explore the role of adrenal gastrinomas in rare endocrine syndromes.
