    Background:

    • Spitz's nevus, a benign melanocytic proliferation, can exhibit atypical features.
    • Partial removal of any nevus carries a risk of recurrence.

    Observation:

    • A case of spindle- and epithelioid-cell nevus (Spitz's nevus) recurrence after incomplete excision is presented.
    • Clinical, histological, and S-100 immunohistochemical findings of the recurrent lesion are detailed.

    Findings:

    • The recurrence presented as a nodular lesion, distinct from the macular recurrence seen in ordinary nevi.
    • Immunohistochemistry confirmed melanocytic origin and growth patterns.

    Implications:

    • Recognizing nodular recurrence of Spitz's nevus is crucial for accurate diagnosis.
    • Distinguishing this recurrence from malignant melanoma prevents unnecessary aggressive treatment.
