    Background:

    • Agoraphobia with panic attacks is a common condition.
    • The dexamethasone suppression test (DST) is used to assess hypothalamic-pituitary-adrenal (HPA) axis function.

    Observation:

    • Six out of 30 agoraphobic subjects with panic attacks had abnormal DST results.
    • Non-suppressor patients showed a trend towards higher rates of major depressive disorders.

    Findings:

    • Abnormal DST results in some agoraphobic patients suggest HPA axis dysregulation.
    • A higher prevalence of major depressive disorders and family history of depression in non-suppressors indicates a potential subtype.

    Implications:

    • Agoraphobia with panic attacks may be a heterogeneous disorder, not solely an anxiety state.
    • Some agoraphobia presentations might be more closely related to endogenous depression, warranting different diagnostic and treatment approaches.
