    • Retinal vascular occlusive diseases are common causes of vision loss.

    Observation:

    • A 58-year-old hypertensive male presented with superior temporal branch retinal vein occlusion.
    • Laser photocoagulation was performed for associated cystoid macular edema.
    • Two years later, the patient developed pre- and sub-retinal hemorrhage from a superior temporal retinal arterial macroaneurysm.

    Findings:

    • Initial fluorescein angiography showed venous occlusion with normal arterial flow.
    • Subsequent angiography revealed a retinal arterial macroaneurysm with normal flow, which later occluded.
    • The case documents the natural evolution of a retinal arterial macroaneurysm.

    Implications:

    • This case contributes to the limited literature on retinal arterial macroaneurysm development and progression.
    • Understanding the natural history of these lesions is crucial for patient management.
    • Further research is needed to elucidate the pathogenesis and optimal treatment strategies.
