    Summary

    Rheumatoid factor (RF) was found in 85% of middle ear effusion (MEE) samples from patients with chronic otitis media with effusion (OME), but only 8% of serum samples. Mucoid MEE samples showed significantly higher RF titers than serous ones.

    Background:

    • Rheumatoid arthritis (RA) and otitis media with effusion (OME) share pathological similarities.
    • Rheumatoid factor (RF) is a key biomarker in RA diagnosis.
    • The presence and significance of RF in OME are not well-established.

    Purpose of the Study:

    • To investigate the presence and levels of rheumatoid factor (RF) in middle ear effusion (MEE) and serum samples from patients with chronic OME.
    • To explore potential correlations between RF in MEE and clinical factors.

    Main Methods:

    • Quantitative latex agglutination test was used to measure RF.
    • 156 human MEE and serum samples from patients with chronic OME were analyzed.
    • RF titers in mucoid and serous MEE samples were compared.

    Main Results:

    • RF was detected in 85% of MEE samples, compared to only 8% of serum samples.
    • RF titers were significantly higher (sevenfold) in mucoid MEE samples than in serous MEE samples.
    • RF presence in MEE was not associated with patient age, bacterial presence, or OME history.

    Conclusions:

    • Rheumatoid factor is frequently present in the middle ear effusions of patients with chronic OME.
    • The high prevalence of RF in MEE suggests a potential role in the pathogenesis of OME.
    • Further research is warranted to understand the implications of RF in OME.
