    Background:

    • Estramustine phosphate, a hybrid steroid-alkylating agent, has been utilized for prostate cancer treatment since 1969.
    • Advanced stages of prostate cancer (Stages C and D) present significant treatment challenges.

    Purpose of the Study:

    • To evaluate the efficacy of estramustine phosphate in patients with Stages C and D prostate cancer.
    • To assess response rates and disease stabilization in patients with varying treatment histories.

    Main Methods:

    • A cohort of 32 patients with Stages C and D prostate cancer received estramustine phosphate treatment.
    • Response was assessed using National Prostatic Cancer Project (NPCP) criteria.
    • Patient subgroups included those with and without prior hormonal manipulation, and varying tumor stages (C, D1, D2).

    Main Results:

    • No complete or partial objective responses were observed.
    • Disease stabilization was achieved in 62% of treatment-naive patients and 12% of those progressing on hormonal therapy.
    • Stabilization durations averaged 14.8 months, with varying rates across stages (Stage C: 100%, D1: 50%, D2: 28%).
    • Tumor grade did not correlate with treatment response.
    • Acid phosphatase levels showed a trend: decline associated with stability, no decline with progression.

    Conclusions:

    • Estramustine phosphate demonstrates efficacy in stabilizing prostate cancer, particularly in patients without prior hormonal therapy.
    • For hormone-refractory prostate cancer, prognosis remains poor, but estramustine phosphate may offer advantages over conventional therapies.
    • Further investigation into estramustine phosphate's role in advanced prostate cancer is warranted.
