    Summary

    Unilateral tinnitus linked to increased intracranial pressure can be caused by venous bruits. Lowering intracranial pressure or applying jugular vein pressure can alleviate this specific type of tinnitus.

    Background:

    • Increased intracranial pressure (ICP) can manifest with various neurological symptoms.
    • Tinnitus, often subjective, can sometimes be linked to underlying physiological mechanisms.
    • Unilateral tinnitus requires thorough investigation to identify its cause.

    Observation:

    • Five patients presented with unilateral tinnitus attributed to elevated intracranial pressure from diverse etiologies.
    • The tinnitus in these cases was characterized as a venous bruit.
    • Patients reported symptom reduction with Valsalva's maneuver, ipsilateral head turning, and ipsilateral jugular vein compression.

    Findings:

    • The tinnitus was directly correlated with increased intracranial pressure.
    • A venous bruit, caused by turbulent blood flow from high-pressure intracranial venous systems to the low-pressure jugular bulb, was identified as the mechanism.
    • Interventions reducing intracranial pressure or venous pressure effectively diminished the tinnitus.

    Implications:

    • This study highlights a specific, treatable cause of unilateral tinnitus.
    • Understanding the mechanism of venous bruits in elevated ICP can guide diagnostic and therapeutic strategies.
    • Further research into vascular causes of tinnitus is warranted.
