    Background:

    • Maffucci syndrome is a rare congenital mesodermal dysplasia characterized by enchondromas and hemangiomas.
    • Patients with Maffucci syndrome have an elevated risk of developing malignant tumors, particularly chondrosarcomas.

    Observation:

    • A case study of a 68-year-old male with Maffucci syndrome experiencing multiple malignant degenerations is presented.
    • Radiological and pathological findings, including bone scintigraphy, were analyzed.

    Findings:

    • Bone scintigraphy revealed distinct findings for two histologically different chondrosarcomatous lesions.
    • Technetium-99m methylene diphosphonate (Tc-99m MDP) uptake appeared to correlate with the degree of calcification within the lesions, irrespective of their benign or malignant nature.

    Implications:

    • This suggests that calcification, rather than malignancy itself, may be a primary factor influencing Tc-99m MDP uptake in Maffucci syndrome.
    • Understanding these scintigraphic patterns can aid in the diagnostic evaluation of Maffucci syndrome patients with suspected malignant transformation.
