    Background:

    Observation:

    • Four male patients presented with oligospermia and infertility.
    • Consistent, mild to moderate elevations in serum prolactin were observed in these patients.
    • Testicular biopsies showed impaired spermatogenesis, from hypospermatogenesis to germ cell absence.

    Findings:

    • No common causes of hyperprolactinemia (e.g., tumors, hypothyroidism, certain medications) were identified.
    • The observed hyperprolactinemia in these patients was idiopathic or unexplained by typical factors.
    • A correlation between elevated prolactin and reduced sperm production was noted.

    Implications:

    • Idiopathic hyperprolactinemia may be an underrecognized cause of male infertility.
    • Further research is warranted to elucidate the mechanisms linking prolactin to spermatogenesis.
    • This finding could lead to new diagnostic and therapeutic approaches for male infertility.
