    Summary

    Painful ophthalmoplegia signaled cavernous sinus infiltration by lymphosarcoma in two patients. Early CT scans were normal, despite later symptomatic remission, the prognosis remained poor.

    Background:

    • Lymphosarcoma can infiltrate the central nervous system.
    • The cavernous sinus is a critical venous structure at the base of the skull.

    Observation:

    • Two cases of lymphosarcoma infiltrating the cavernous sinus presented with painful ophthalmoplegia.
    • Symptom onset and disease progression varied between the two cases.
    • Computed tomography (CT) scans were normal in the early disease stages.

    Findings:

    • Painful ophthalmoplegia served as the initial clinical sign of cavernous sinus involvement.
    • CT imaging, a key diagnostic tool for lymphomas, failed to detect early-stage infiltration.
    • Treatment led to symptomatic remission in both patients.

    Implications:

    • Early diagnosis of cavernous sinus lymphomas may be challenging due to normal initial imaging.
    • Despite treatment, the poor prognosis for cavernous sinus lymphosarcoma necessitates further research.
    • This highlights the importance of clinical suspicion in diagnosing rare oncological emergencies.
