    Background:

    • Malignant lymphoma primarily affecting the female genital tract is a rare clinical entity.
    • It accounts for a small percentage of ovarian (0.3%) and uterine (0.12%) malignancies.

    Observation:

    • Four cases highlight diagnostic challenges in differentiating lymphoma from common gynecologic cancers.
    • Uterine lymphoma often presents as cervical carcinoma, while ovarian lymphoma can be bilateral and mimic other ovarian tumors.
    • Histological features can be ambiguous, leading to misdiagnosis, especially in frozen sections.

    Findings:

    • Rapidly developing, undifferentiated round-cell tumors in the uterus suggest lymphoma.
    • Bilateral ovarian tumors without ascites and unclear histology warrant consideration of lymphoma.
    • The diffuse, poorly differentiated lymphocytic type is common in adults; granulocytic sarcoma in children.

    Implications:

    • Establishing diagnostic criteria is crucial for clinicians and pathologists.
    • Specialized morphologic and immunologic investigations are recommended for ambiguous cases.
    • Timely diagnosis of primary genital tract lymphoma improves patient management and outcomes.
