    Summary

    Hypertension and arterial wall damage, possibly from emboli or occlusive disease, may lead to retinal arterial aneurysms, particularly in older patients. These aneurysms show characteristic wall thickening and thrombus formation.

    Background:

    • Retinal arterial aneurysms are rare vascular lesions.
    • Hypertension is a known risk factor for vascular disease.

    Observation:

    • Postmortem examination revealed retinal arterial aneurysms in two hypertensive patients.
    • Histological analysis showed thickened vessel walls with hyaline, fibrin, and macrophages, and thrombus within the aneurysms.

    Findings:

    • Atherosclerosis was noted in larger arterial branches.
    • Progressive aneurysmal changes, from "cuff" to hemorrhagic types with vessel wall splitting, were observed.
    • Fat deposition was present in most aneurysmal walls.

    Implications:

    • Arterial wall damage, potentially from cholesterol emboli or occlusive disease, may predispose hypertensive individuals to retinal aneurysm formation.
    • These findings highlight a potential mechanism for retinal aneurysm development in at-risk populations.
