    Background:

    • Internal hernias are rare, often presenting with non-specific symptoms.
    • Herniation through the gastrohepatic ligament is exceptionally uncommon.
    • Gastric outlet obstruction typically results from peptic ulcer disease or malignancy.

    Observation:

    • A patient presented with symptoms suggestive of gastric outlet obstruction.
    • Imaging revealed an internal hernia involving the small bowel through the gastrohepatic ligament.
    • The herniated small bowel was causing extrinsic compression on the stomach.

    Findings:

    • This is the first reported case of a gastrohepatic ligament hernia causing gastric outlet obstruction.
    • The obstruction was secondary to the herniated small bowel compressing the stomach, not the small bowel itself.
    • Successful surgical reduction of the hernia and relief of symptoms were achieved.

    Implications:

    • This case expands the differential diagnosis for gastric outlet obstruction.
    • It underscores the importance of considering rare internal hernias in patients with unexplained upper gastrointestinal symptoms.
