    Summary

    Intraincisional ampicillin instillation in laparotomy wounds achieved high concentrations in wound secretions for 8 hours post-surgery. Further clinical trials are recommended to evaluate its efficacy in preventing wound sepsis.

    Background:

    • Surgical site infections (SSIs) remain a significant concern, particularly following abdominal surgery.
    • Systemic antibiotic prophylaxis is standard, but localized wound concentrations may be suboptimal.
    • Intraincisional antibiotic instillation offers a targeted approach to enhance local drug levels.

    Purpose of the Study:

    • To measure ampicillin concentrations in wound secretions after intraincisional instillation during appendectomy.
    • To assess the duration and magnitude of local ampicillin levels.
    • To provide preliminary data for a potential clinical trial on intraincisional antibiotics for colorectal surgery.

    Main Methods:

    • Ten patients undergoing appendectomy received 1 gm ampicillin under the fascia and 1 gm in the subcutaneous space.
    • Wound secretions were collected via drain every 2 hours for 24 hours post-operation.
    • Ampicillin concentrations were determined using a disk diffusion method.

    • Median ampicillin concentrations exceeded 1000 microgram/ml in wound secretions for the first 8 hours.

    • These findings support the need for a clinical trial to investigate intraincisional antibiotics as an adjunct to systemic prophylaxis in colorectal surgery.
    • Evaluating the impact on wound sepsis is crucial for optimizing surgical site infection prevention strategies.
