    Summary

Total hip replacement (THR) in children with juvenile rheumatoid arthritis and ankylosing spondylitis yielded encouraging results. All patients experienced improved range of motion and pain relief, suggesting THR is a viable option for pediatric hip disease.

    Area of Science:

    • Orthopedic surgery
    • Pediatric rheumatology

    Background:

    • Juvenile rheumatoid arthritis and ankylosing spondylitis can cause severe hip disease in children.
    • Limited treatment options exist for severe pediatric hip conditions, often leading to significant pain and functional limitations.

    Purpose of the Study:

    • To evaluate the outcomes of total hip replacement (THR) in pediatric patients with inflammatory arthropathies.
    • To assess the safety and efficacy of THR in managing severe hip disease in children and adolescents.

    Main Methods:

    • Retrospective review of 29 total hip replacements (THR) performed on 16 children.
    • Patients included those with juvenile rheumatoid arthritis and ankylosing spondylitis, aged 12-18 years.
    • Follow-up ranged from 1 to 4 years (median 2.2 years).

    Related Experiment Videos

    Main Results:

    • All 29 hips showed improved postoperative range of motion.
    • All patients were pain-free after surgery.
    • Primary indications included severe malposition/flexion contractures (24 hips) and pain (5 hips).

    Conclusions:

    • Total hip replacement (THR) in children with juvenile rheumatoid arthritis and ankylosing spondylitis demonstrates encouraging outcomes.
    • Improved range of motion and pain relief were consistently observed.
    • Considerations for THR in pediatric patients include active rheumatic disease, prosthesis longevity, and patient motivation.
